pathways connect 50 plus

Pathways Connect 50 Plus provides case-managed support to clients who are 50 years and over who require monitoring of their health and well being and support to access and maintain public housing.

Pathways Connect 50 Plus clients may have unmet needs associated with conditions such as frailty due to age, mental illness, problematic alcohol and substance abuse issues, multiple disabilities, chronic health needs and social isolation. Clients also have problems maintaining stable accommodation and accessing support services.

Pathways Connect 50 Plus provides
• Flexible, case managed support on an outreach basis (short, medium or long term)
• Care coordination, including linkages and referrals, to appropriate services to assist with health, recreation, advocacy, employment and education needs
• Support and linkages to assist with living skills including budgeting, shopping and attending appointments
• Regular group activities

This program has been specifically designed for older people who are about to enter direct tenure public housing through the priority segments of the Office of Housing Segmented Waiting List. The Program also supports current Office of Housing residents.
We aim to
• Assist clients to maintain stable, long-term housing
• Improve clients’ health and wellbeing
• Encourage and support clients to make decisions regarding their use of services, and manage their health and welfare needs.

We establish supportive relationships with clients and arrange flexible service responses to ensure clients receive effective assistance.

Referrals to Pathways Connect 50 Plus are made through Ermha’s Pathways Connect program. Referrals are also made by the Victorian Government’s Office of Housing and community housing providers. We also welcome self-referrals.
