What is an urgently reportable NDIS incident?
An ISR1 reportable incident indicates a severe adverse outcome or death. These incidents are considered the highest severity category and require specific reporting and investigation procedures at ermha365. We must notify the NDIS Commission of all reportable incidents (including allegations), even when we have acted and responded appropriately.
These six categories are ALL classified as ISR1 incidents:
Death of a person with disability
All deaths of people with disability that occur in connection with the provision of NDIS supports or services must be reported.
Serious injury of a person with disability
Fractures, burns, deep cuts, extensive bruising, head or brain injuries which might be indicated by concussion or loss of consciousness, or any other injury requiring hospitalisation.
Abuse or neglect of a person with disability
Abuse includes physical abuse, psychological or emotional abuse, and financial abuse.
Neglect includes grossly inadequate care, failure to access medical care, supervisory neglect, a reckless act or failure to act, failure to protect from abuse.
Unlawful sexual or physical contact with, or assault of, a person with disability
Physical assaults include hitting, pushing, shoving, spitting, throwing objects towards other people or making threats of physical harm.
Unlawful sexual contact or assault encompasses any behaviour of a sexual nature that is an offence under any criminal statute of a state, territory or the Commonwealth.
Sexual misconduct against, or in the presence of, a person with disability, including grooming of the person for sexual activity
Sexual misconduct includes unlawful sexual conduct; sexually explicit comments and overtly sexual behaviour; crossing professional boundaries in a way that has sexual implications or connotations; grooming of the person for sexual activity.
Use of a restrictive practice that is unauthorised by your state or territory, or does not follow a behaviour support plan for the person with disability
Involves the use of practices that have the effect of restricting the rights or freedom of movement of a person with disability. This can include restraint (chemical, mechanical, social or physical) and seclusion (keeping someone in isolation).
What do I do now?
Staff roles and responsibilities
Staff Member
1. Call immediate supervisor and provide details as soon as is safe or practicable.
If after hours, call IHL.
2. Enter the incident on RiskMan within 4 hours of it occurring.
Team or Practice Leader or IHL Tier 1
1. Review all incidents entered into RiskMan and identify any incidents that are reportable.
2. Immediately inform ermha365 Key Personnel of the incident by phone.
3. Enter all known information into Provider Portal within 12 hours of being made aware of the incident.
If after hours, Tier 2 IHL enters information into Provider Portal.
Key Personnel
Review incident and if correct, approve and submit the report on Provider Portal within 24 hours of the incident.
If after hours, Tier 2 IHL approves the external report within 24 hours of the incident.
Key Personnel include Tier 2 IHL, Senior Managers, General Managers and Executives.
